Regeneron and Vyriad Announce Strategic Agreement for Discovery and Development of New Oncolytic Virus Treatments for Cancer

On November 6, 2019 Regeneron Pharmaceuticals, Inc. (NASDAQ: REGN) and Vyriad, Inc. reported a research collaboration and option licensing agreement focused on the development of new oncolytic (cancer-killing) virus-based treatments for various forms of cancer (Press release, Vyriad, NOV 6, 2019, View Source [SID1234550559]). The agreement includes a Phase 2 clinical study, slated to begin in 2020, evaluating Regeneron’s PD-1 inhibitor Libtayo (cemiplimab-rwlc) in combination with Vyriad’s oncolytic virus Voyager-V1 in multiple types of cancer, including melanoma, lung, liver and endometrial cancers. Libtayo is a fully-human monoclonal antibody targeting the immune checkpoint receptor PD-1 (programmed cell death protein-1) and is being jointly developed and commercialized by Regeneron and Sanofi under a global collaboration agreement. Libtayo was invented by Regeneron using the company’s proprietary VelocImmune technology which uses a unique genetically-humanized mouse to produce optimized fully-human antibodies.

Vyriad’s investigational drug candidate Voyager-V1 is a potent VSV programmed to attack cancer cells selectively, while also activating the immune system to kill local and distant cancer cells. Further, it amplifies inflammatory and antitumor immune system responses that help turn "cold" tumors "hot," which potentially enhances anti-PD-1 activity. Voyager-V1 is deliverable by intravenous infusion.

Under the agreement, Vyriad will receive an upfront payment and Regeneron will make an equity investment in the company. Regeneron will have an exclusive option to license Voyager-V1 and other collaboration products. Vyriad is eligible to receive additional payments based on the achievement of specified development and commercial milestones, as well as royalties on net sales of potential future VSV-based collaboration products. During the five-year collaboration term, Vyriad will work exclusively with Regeneron to research and develop VSV technologies. Specific financial terms were not disclosed.

Voyager-V1 is an investigational oncolytic virus that was designed for enhanced safety, efficacy, and traceability through the inclusion of an interferon beta gene, enabling the virus to replicate quickly in cancer cells without damaging healthy cells, recruit cancer-fighting immune cells to the tumor, and secrete a measurable reporter protein into the blood. Voyager-V1 was also engineered to include an iodine transporter NIS gene that facilitates tracking of the virus’ spread to cancer cells throughout the body. Libtayo is approved in the U.S., European Union, Canada and Brazil for adult patients with metastatic cutaneous squamous cell carcinoma (CSCC) or locally advanced CSCC who are not candidates for curative surgery or curative radiation. Libtayo is a medicine that may treat a type of skin cancer by working with your immune system. Libtayo can cause your immune system to attack normal organs and tissues in any area of your body and can affect the way they work. These problems can sometimes become severe or life-threatening and can lead to death. These problems may happen anytime during treatment or even after your treatment has ended.

Call or see your healthcare provider right away if you develop any symptoms of the following problems or these symptoms get worse:

Lung problems (pneumonitis). Signs and symptoms of pneumonitis may include new or worsening cough, shortness of breath, and chest pain.
Intestinal problems (colitis) that can lead to tears or holes in your intestine. Signs and symptoms of colitis may include diarrhea (loose stools) or more frequent bowel movements than usual; stools that are black, tarry, sticky or that have blood or mucus; and severe stomach-area (abdomen) pain or tenderness.
Liver problems (hepatitis). Signs and symptoms of hepatitis may include yellowing of your skin or the whites of your eyes, severe nausea or vomiting, pain on the right side of your stomach area (abdomen), drowsiness, dark urine (tea colored), bleeding or bruising more easily than normal, and feeling less hungry than usual.
Hormone gland problems (especially the adrenal glands, pituitary, thyroid and pancreas). Problems in other organs. Infusion (IV) reactions that can sometimes be severe and life-threatening. Signs of these problems may include chills or shaking, itching or rash, flushing, shortness of breath or wheezing, dizziness, fever, feeling of passing out, back or neck pain, and facial swelling

Getting medical treatment right away may help keep these problems from becoming more serious.

have immune system problems such as Crohn’s disease, ulcerative colitis, or lupus;
have had an organ transplant;
have lung or breathing problems;
have liver or kidney problems;
have diabetes;
are pregnant or plan to become pregnant; Libtayo can harm your unborn baby.
Females who are able to become pregnant:

Your healthcare provider will give you a pregnancy test before you start treatment.
You should use an effective method of birth control during your treatment and for at least 4 months after your last dose of Libtayo. Talk with your healthcare provider about birth control methods that you can use during this time.
Tell your healthcare provider right away if you become pregnant or think you may be pregnant during treatment with Libtayo.

Q3 2019 Highlights

Announced extension of a strategic collaboration with the National Center for Complementary and Integrative Health (NCCIH) at the National Institutes of Health (NIH). As part of the collaboration, the NIH is funding and providing research efforts to conduct additional non-clinical development as requested by the FDA to support cytisinicline’s New Drug Application (NDA).

Presented positive efficacy and compliance results from the Phase 2b ORCA-1 dose-selection trial evaluating cytisinicline in 254 smokers at the Society for Research on Nicotine & Tobacco Europe (SRNT-E) 19th Annual Conference. Cytisinicline demonstrated a statistically significant improvement in quit rates for a simplified 3.0 mg, three times daily dose and was well-tolerated with no serious adverse events reported.

Hosted Company’s first Investor Day, including roundtable discussion with smoking cessation medical experts and presentation of final ORCA-1 data, including plans for Phase 3 development program.

Extended Collaboration with NIH

In July, Achieve announced an extension of its strategic collaboration with the National Center for Complementary and Integrative Health (NCCIH) at the National Institutes of Health (NIH). Under the extended collaboration, the NIH is funding and providing research efforts to conduct additional non-clinical studies as requested by the U.S. Food and Drug Administration (FDA) to be included in a cytisinicline New Drug Application (NDA). In discussions with the FDA in 2018, it was determined that a Good Laboratory Practice (GLP) non-clinical reproductive study would be required to support the NDA submission. NCCIH has agreed to sponsor this study which is expected to complete in 2020. In total, the NIH has committed approximately $6 million dollars to the development of cytisinicline.

Presented Final ORCA-1 Data at SRNT-E Conference

Achieve’s Chief Scientific Officer, Dr. Anthony Clarke, facilitated two oral presentations featuring data from the Phase 2b ORCA-1 trial at the SRNT-E Annual Conference in Oslo. The ORCA-1 trial of cytisinicline in 254 U.S. smokers demonstrated a statistically significant improvement in quit rates for the 3.0 mg, three times daily dosing (TID) schedule. In the 3.0 mg TID arm, a 54% abstinence rate at week 4, compared to 16% for placebo (p < 0.0001) was observed. Continuous abstinence at weeks 5 through 8 was 30% for cytisinicline compared to 8% for placebo (p= 0.005). Adherence to study treatment was 98% in the 3.0 mg TID arm and cytisinicline was well-tolerated with no serious adverse events reported.

Key findings of the abstract include:

ARQ 531 continues to be well-tolerated through 65 mg QD and has a manageable safety profile in multiple B-cell malignancies
Pharmacokinetic (PK) data show that patients receiving 65 mg QD of ARQ 531 exhibited steady-state mean Cmin of above 1 µM, with complete pBTK inhibition
Robust, dose-dependent, anti-tumor activity was observed, including 10 PRs, especially at the higher doses
Of the 6 evaluable patients recruited in cohort 7 with R/R CLL/SLL and dosed initially at 65 mg QD, 5 experienced a PR as of July 19, 2019
Two additional R/R CLL patients experienced a PR: 1 patient dose escalated from 45 to 65 mg QD and another de-escalated from 75 to 65 mg QD
Three additional PRs were observed outside of CLL including 1 patient with Follicular Lymphoma, 1 with Richter’s Transformation and 1 with Diffuse Large B-Cell Lymphoma

Bruton’s tyrosine kinase, BTK, is a therapeutic target that has been clinically proven to inhibit B-cell receptor signaling in blood cancers. IMGN632 is a CD123-targeting ADC in Phase I testing for hematological malignancies, including acute myeloid leukemia (AML), blastic plasmacytoid dendritic cell neoplasm (BPDCN), and acute lymphocytic leukemia (ALL). IMGN632 uses one of ImmunoGen’s novel indolino-benzodiazepine (IGN) payloads, which alkylate DNA without crosslinking. IGNs have been designed to have high potency against AML blasts, while demonstrating less toxicity to normal marrow progenitors than other DNA-targeting payloads.

Updated safety and efficacy findings from the dose escalation and expansion of the first-in-human trial of IMGN632 in patients with relapsed/refractory AML and BPDCN will be reported in an oral presentation. Preclinical data related to IMGN632 in combination with Vidaza (azacitidine) and Venclexta (venetoclax) and two "trial in progress" posters will also be presented in poster sessions.

IMGN632 was administered to 74 patients over dose levels ranging from 0.015 to 0.45 mg/kg intravenously on the every 3 week schedule and 0.015-0.06 mg/kg on the fractionated day 1, 4, and 8 schedule every 3 weeks.
IMGN632 displays a tolerable safety profile and activity at doses up to 0.3 mg/kg.
The most common treatment-related adverse event was infusion-related reactions (16%; four grade 3); none required treatment discontinuation.
Single dose-limiting toxicities were seen at the three highest dose levels tested: one prolonged neutropenia and two reversible cases of veno-occlusive disease; no patterns of hepatotoxicity or cytopenias occurred with doses below 0.18 mg/kg.
Although no maximum tolerated dose was determined on either schedule, based on the efficacy, safety, and pharmacokinetic data generated, the dose and schedule of 0.045 mg/kg given on day 1 every 3 weeks has been selected for Phase 2 development.
AML Efficacy

In the assessable AML population (n=66), 37 (55%) had a reduction in bone marrow blasts and 13 (20%) achieved an objective response across all dose levels and both schedules achieved an objective response, including three complete remissions (CR) and eight CRs with incomplete recovery (CRi) in heavily pretreated patients. The majority of responders (77%) had failed prior intensive therapies, including three with prior transplant, 62% had an adverse risk classification, and 23% were primary refractory.
A 32% response rate (6/19 patients; two CR, three CRi, and one morphologic leukemia free state) was seen in primary AML patients treated with dose and schedule selected for Phase 2 development.
BPDCN Efficacy

Three of seven evaluable BPDCN patients (43%) achieved a response after a single dose of IMGN632, one CR, one CRi, and one partial remission; all three patients had received prior SL-401 (tagraxofusp-erzs; Elzonris).

ABOUT ACUTE MYELOID LEUKEMIA (AML)
AML is a cancer of the bone marrow cells that produce white blood cells. It causes the marrow to increasingly generate abnormal, immature white blood cells (blasts) that do not mature into effective infection-fighting cells. The blasts quickly fill the bone marrow, impacting the production of normal platelets and red blood cells. The resulting deficiencies in normal blood cells leave the patient vulnerable to infections, bleeding problems, and anemia. It is estimated that, in the U.S. alone, 21,380 patients will be diagnosed with AML this year and 10,590 patients will die from the disease.

ABOUT BLASTIC PLASMACYTOID DENDRITIC CELL NEOPLASM (BPDCN)
BPDCN is a rare form of blood cancer that has features of both leukemia and lymphoma, with characteristic skin lesions, lymph node involvement, and frequent spread to the bone marrow. This aggressive cancer requires intense treatment often followed by stem cell transplant. Despite the recent approval of a CD123-targeting therapy, the unmet need remains high in the relapsed/refractory setting.

ABOUT CD123
CD123, the interleukin-3 alpha chain, is expressed on multiple myeloid and lymphoid cancers including AML, BPDCN, ALL, chronic myeloid leukemia, and myeloproliferative neoplasms. With limited expression on normal hematopoietic cells, rapid internalization, and expression on AML leukemia stem cells, CD123 is a validated therapeutic target, with the recent approval of a CD123-targeting therapy for BPDCN.
