Platform Technologies Update

XCART Technology

On July 19, 2019, the Company completed its previously announced acquisition of a novel CAR T platform technology, called "XCART." XCART is a proximity-based screening platform capable of identifying CAR constructs that can target patient-specific tumor neoantigens, with a demonstrated proof of mechanism in B-cell Non-Hodgkin lymphomas. The XCART technology, developed by The Scripps Research Institute in collaboration with the Shemyakin-Ovchinnikov Institute of Bioorganic Chemistry, is believed to have the potential to significantly enhance the safety and efficacy of cell therapy for B-cell lymphomas by generating patient- and tumor-specific CAR T cells.

The XCART technology platform was designed by its originators to utilize an established screening technique to identify peptide ligands that bind specifically to the unique B-cell receptor ("BCR") on the surface of an individual patient’s malignant tumor cells. The peptide is then inserted into the antigen-binding domain of a CAR, and a subsequent transduction/transfection process is used to engineer the patient’s T cells into a CAR T format which redirects the patient’s T cells to attack the tumor. Essentially, the XCART screening platform is the inverse of a typical CAR T screening protocol wherein libraries of highly specific antibody domains are screened against a given target. In the case of XCART screening, the target is itself an antibody domain, and hence highly specific by its nature. The XCART technology creates the possibility of personalized treatment of lymphomas utilizing a CAR with an antigen-binding domain that should only recognize, and only be recognized by, the unique BCR of a particular patient’s B-cell lymphoma.

An expected result for XCART is reduced off-tumor toxicities typically associated with CAR T lymphoma therapies, such as B-cell aplasia. Xenetic’s clinical development program will seek to confirm the early preclinical results, and to demonstrate a more attractive safety profile than existing therapies.

Xenetic recently entered into a research agreement to begin the process of technology transfer of the XCART technology and enable advancement towards Xenetic’s stated goal of establishing an academic collaboration for XCART development. The Company’s early activities will build off of the work of the XCART inventors which was previously described in a Science Advances article published in November 2018.[2]

Dr. Guenther Koehne, Chief of Blood & Marrow Transplant and Hematologic Oncology at the Miami Cancer Institute, commented, "At this point, the power of CAR T therapy has been established, especially in the area of hematological malignancies. However, as the level of activity in the CAR T space suggests, there remains substantial unmet need in oncology treatments as existing therapies can be highly efficacious but not all patients respond, and they can often induce serious, even life-threatening side effects. PolyXen Platform Technology

The Company’s proprietary drug development platform, PolyXen, is a platform technology which can be applied to protein or peptide therapeutics, enabling next-generation biological drugs to prolong a drug’s circulating half-life and potentially improve other pharmacological properties. PolyXen has been demonstrated in human clinical trials to confer prolonged half-life on biotherapeutics such as recombinant human erythropoietin and recombinant Factor VIII ("rFVIII"). Additionally, the Company believes that PolyXen has potential utility in other molecule classes such as small molecules.

Xenetic and its partners incorporate the Company’s patented and proprietary PolyXen technology into drug candidates currently under development with biotechnology and pharmaceutical industry collaborators to create what the Xenetic believes will be the next-generation biologic drugs with improved pharmacological properties over existing therapeutics. Xenetic currently has an exclusive license agreement with Takeda Pharmaceuticals Co. Ltd. ("Takeda") in the field of coagulation disorders, under which Takeda currently has one active development program underway utilizing the PolyXen platform technology. In addition, in October 2017, Xenetic granted rights to Takeda to grant a nonexclusive sublicense to certain patents related to PolyXen to a third party. The Company expects to receive royalties as a result of the sublicense agreement as early as the fourth quarter of this year.

The paper entitled "First-in-human imaging with 89Zr-Df-IAB22M2C anti-CD8 minibody in patients with solid malignancies: preliminary pharmacokinetics, biodistribution, and lesion targetingi" reports the results from a first in-vivo clinical study to assess the safety, tolerability, and efficacy of ImaginAb’s lead product, CD8 tracer, 89Zr-Df-IAB22M2C, in visualizing the immune system.

The data from the Phase I trial reported in the paper demonstrated 89Zr-Df-IAB22M2C was well tolerated with no immediate or delayed side effects observed. In conclusion, the study found 89Zr- Df-IAB22M2C imaging to be safe and with favorable kinetics for early imaging. The biodistribution suggested successful targeting of CD8+ T cell-rich tissues. The observed targeting of tumor lesions suggests this may be informative for CD8+ T cell accumulation within tumors.

Lead author of the study, Neeta Pandit-Taskar, MD, Nuclear Medicine Physician at Memorial Sloan Kettering Cancer Center, said: "This novel imaging agent has the potential to non-invasively assess the presence of CD8 T cells in patients’ tumors and results of this initial assessment are encouraging. With more research, this technology may ultimately serve a critical role as a biomarker of immunotherapy outcome and inform clinical trials of novel immunotherapies that act mechanistically through the presence of CD8 T cells."

GC4419, a highly selective and potent small molecule dismutase mimetic, is being developed to reduce the incidence and severity of radiation-induced severe oral mucositis (SOM), its lead indication. SOM is a common, debilitating complication of radiotherapy in patients with head and neck cancer. There is currently no FDA-approved drug to treat SOM in patients with head and neck cancer.

As part of its Phase 2b clinical trial of GC4419 in patients with locally advanced head and neck cancer, Galera assessed tumor outcomes of the patients over a two-year period following radiotherapy. Patients in the trial received seven weeks of radiation therapy plus cisplatin, and were treated with either 30 mg or 90 mg of GC4419 or placebo by infusion on the days they received their radiation treatment. At both the one-year interim assessment and final two-year mark, tumor outcomes were maintained across all four measures – overall survival, progression-free survival, locoregional control and metastasis-free survival – in both GC4419 dose groups (30 mg and 90 mg) compared to placebo.

"We are pleased with these data, which demonstrated GC4419, when added to a standard radiotherapy regimen, maintained the efficacy of treatment for head and neck cancer and reduced debilitating radiation-induced oral mucositis," said Mel Sorensen, M.D., President and CEO of Galera Therapeutics. "GC4419 achieved meaningful reductions in the duration, incidence and severity of SOM in the completed Phase 2b trial. These two-year tumor data further reinforce the potential of GC4419 to be a promising treatment to reduce radiation toxicities and complement standard radiotherapy regimens in head and neck cancer."

Full results will be submitted for presentation at a future scientific meeting.

GC4419 is being evaluated in the ongoing pivotal ROMAN Phase 3 trial in patients with head and neck cancer, with topline data anticipated in the first half of 2021. GC4419 is also currently being studied in combination with stereotactic body radiation therapy for its anti-tumor effect in a pilot Phase 1b/2a trial of patients with locally advanced pancreatic cancer.

About GC4419 (Avasopasem Manganese)

Galera’s lead product candidate, GC4419 (avasopasem manganese), is a highly selective and potent small molecule dismutase mimetic that is being developed for the reduction of SOM in patients with head and neck cancer. GC4419 is designed to rapidly convert superoxide to hydrogen peroxide, reducing mucosal damage and thereby the incidence and severity of mucositis. Left untreated, elevated superoxide can damage noncancerous tissues and lead to debilitating side effects, including oral mucositis, which can limit the anti-tumor efficacy of radiation therapy.

GC4419 is being studied in the Phase 3 ROMAN clinical trial of patients with locally advanced head and neck cancer, its lead indication, for its ability to reduce the duration, incidence and severity of radiation-induced severe oral mucositis. In Galera’s 223-patient, double blind, randomized, placebo- controlled Phase 2b clinical trial, GC4419 demonstrated the ability to reduce the median duration of severe oral mucositis (SOM) from 19 days to 1.5 days (92 percent), the incidence of SOM through completion of radiation by 34 percent and the severity of patients’ OM by 47 percent, and GC4419 was well tolerated in the trial when added to a standard radiotherapy regimen. GC4419 is also currently being studied in combination with SBRT for its anti-tumor effect in a pilot Phase 1b/2a trial of patients with locally advanced pancreatic cancer. In addition, in multiple preclinical studies, GC4419 demonstrated an increased tumor response to radiation therapy while preventing toxicity in normal tissue.

The U.S. Food and Drug Administration granted Breakthrough Therapy and Fast Track designations to GC4419 for the reduction of the duration, incidence and severity of SOM induced by radiotherapy in patients with head and neck cancer.
