The oral presentation, entitled: "Interim Evaluation of a Targeted Radiotherapeutic, CLR 131, in Relapsed/Refractory Diffuse Large B-Cell Lymphoma Patients (R/R DLBCL)," featured data from 6 subjects who received a single 30-minute intravenous (IV) dose of 25mCi/m2 of CLR 131. Data showed durable responses, including a 33% overall response rate (ORR), a 16.6% complete response rate (CR) and a 50% clinical benefit rate (CBR). All patients enrolled in the study received an average of 3 prior lines of systemic therapy, 5 of 6 patients were refractory to at least one prior line of therapy. Importantly, CLR 131 showed activity against both germinal center and activated DLBCL. In a patient for whom cytogenetics was available, CLR 131 showed activity against c-Myc and BCL-2 mutation (single & dual-hit) positive patients. As required by the Lugano Criteria for Response, the patient who experienced a CR had a total reduction in tumor volume of greater than 99% and continues to be a CR at 510+ days post dosing. This patient was refractory to two prior treatment lines, which included the combination regimen RICE.

Analysis of dosing showed that the disease control rate and progression free survival were markedly improved in patients receiving a dosing ratio of 1.2% or greater (drug to tumor volume) versus those receiving below a 1.2% dosing ratio. Finally, the most frequent adverse events (AE) in DLBCL patients were consistent with prior studies of CLR 131; the majority of AEs being hematologic in nature and predominately Grades 1 and 2.

"The data presented show an encouraging overall response rate including a complete response at the time of the interim assessment after a single 30-minute IV dose of 25mCi/m2 of CLR 131. We remain optimistic that CLR 131 has the potential to provide a meaningful treatment option for a variety of lymphoma patients and look forward to reporting additional data from this Phase 2 CLOVER-1 study in 2019," said James Caruso, president and CEO of Cellectar. "The study remains ongoing and, based on this cohort and additional data from an ongoing dose escalation Phase 1 study, patients in the Phase 2 CLOVER-1 study are now receiving a higher 37.50 mCi/m2 fractionated dose administered in two 30-minute infusions of 18.75mCi/m2."

About the Phase 2 CLOVER-1 Trial

CLOVER-1 is a Phase 2 study of CLR 131 being conducted in approximately 10 leading cancer centers in the United States in patients with relapsed or refractory B-cell hematologic cancers. The hematologic cancers being studied in the trial include multiple myeloma (MM), chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL), lymphoplasmacytic lymphoma (LPL), marginal zone lymphoma (MZL), mantle cell lymphoma (MCL), and diffuse large B-cell lymphoma (DLBCL).

The study will enroll up to 80 patients. Its primary endpoint is clinical benefit response (CBR), with additional endpoints of overall response rate (ORR), progression free survival (PFS), median overall survival (OS) and other markers of efficacy following a fractionated dose of 37.575mCi/m2 of CLR 131 administered in two 30-minute infusions of 18.75mCi/m2 of CLR 131 administered on day 1 and day 8, with the option for a second dose cycle approximately 75-180 days later. The company expects to report topline data in 2019.

About CLR 131

CLR 131 is a small-molecule, cancer-targeting radiotherapeutic Phospholipid Drug ConjugateTM (PDC) designed to deliver cytotoxic radiation directly and selectively to cancer cells and cancer stem cells. CLR 131 is the company’s lead therapeutic PDC product candidate and is currently being evaluated in both Phase 2 and Phase 1 clinical studies. Titled, "Severe oral mucositis (SOM) mitigation by genetically modified Lactococcus lactis bacteria (LLB) producing human trefoil factor 1 (hTFF1; AG013) in patients being treated with concomitant chemoradiation (CRT) for oral and oropharyngeal cancers (OCOPC)," was presented at the ESMO (Free ESMO Whitepaper) Congress 2019 in Barcelona, Spain. The poster presentation describes the methods and initial blinded results from the ongoing Phase 2 clinical trial for the Company’s lead oral mucositis product candidate, AG013. The ongoing Phase 2 clinical trial is a double-blind, placebo-controlled, two-arm, multi-center trial, in which approximately 200 patients will be randomized in a 1:1 ratio to receive either AG013 or placebo three times daily following meals, beginning on the first day of chemoradiation therapy and continuing through the course of cancer treatment. The purpose of this Phase 2 clinical study, (NCT03234465), is to evaluate the efficacy (preventing the occurrence and shortening the duration of SOM), safety, and tolerability of a convenient topically administered rinse of AG013 compared to a placebo for reducing the incidence and severity of oral mucositis in patients undergoing traditional chemoradiation for the treatment of head and neck cancer. The initial data, submitted in the abstract, reflects the results for 42 of the 71 enrolled and randomized patients across 48 study sites and demonstrates that in the blinded, combined placebo and active treatment groups, there was sufficient evidence of efficacy and safety to continue the study.

Additional data accumulated since poster submission, indicates the blinded efficacy evaluation, which included any patient with SOM after week one of treatment and those receiving a cumulative dose of 55 Gy (week 6 of treatment), demonstrated an overall SOM incidence of 47%, which is lower than would be expected based on historical data in the head and neck cancer population receiving this chemoradiation regimen. The overall rate of SOM was reported in only 13.1 % (110 of 842) of evaluable visits. The overall safety profile is consistent with those adverse events that normally occur in cancer patients receiving chemoradiation therapy. As a reminder, the study remains blinded and individual treatment responses remain to be identified. The lead author for the poster presentation is Suraj Singh, M.D., of the MultiCare Regional Cancer Center in Tacoma, Washington.

About Oral Mucositis

Oral mucositis is currently one of the most common and debilitating complications of cancer chemo- and radiation therapy. The condition is caused by the breakdown of the mucosal lining in the oral cavity resulting in the formation of painful mouth ulcers. When these mouth ulcers progress to World Health Organization (WHO) grade 3 and 4, patients by definition, have their ability to eat (grade 3) and drink (grade 4) impacted resulting in emergency room visits or hospitalization in order to provide pain control and nutritional support. During these periods, patients run the risk of interruption of their chemo- and radiation therapies with the potential risk of negative cancer treatment outcomes. The incidence of SOM is approximately 70% in oropharyngeal cancer patients.

AG013, which has been granted Fast Track designation with the U.S. Food and Drug Administration and orphan drug status in Europe, is an ActoBiotics therapeutic candidate formulated to deliver the therapeutic molecule Trefoil Factor 1 to the mucosal tissues in the oral cavity in a convenient oral rinsing solution. The published data comes from the development phase of INDUCE-1, the first open-label study in humans to study GSK3359609 as a monotherapy and a combination of other regimens. The patients in the study had relapsed or metastatic head and neck squamous cell carcinoma, and had previously received up to five therapies in the late stages. Patients in the monotherapy group were previously treated with PD-1/L1 and 1 mg/kg GSK3359609. Patients in the combination drug group received PD-1/L1 treatment for the first time, taking 0.3 mg/kg GSK3359609 and 200 mg pemizumab. Patients in both groups received a two-year assessment until the condition progressed or unacceptable toxicity occurred.

Of the 34 patients who received combination therapy, the overall response rate was 24% (n=8; 95% CI: 11,58.7). Remission in the combination drug group was long-lasting, and all responders maintained a 6-month or longer improvement (no median; 95% CI: 4.2 months, NR); median progression-free survival (PFS) ) for 5.6 months (95% CI: 2.4, 7.4). Among the 21 patients with known PD-L1 expression data, most responders and stable patients had a PD-L1 score of less than 20 points. Among the 16 patients who underwent monotherapy, the overall response rate was 6% (n = 1; 95% CI: 0.2, 30.2).

The INDUCE-1 study was conducted in accordance with an agreement between GlaxoSmithKline and Merck & Co, Inc. ("MSD" outside the United States and Canada). GlaxoSmithKline will continue to work with MSD to support the INDUCE-3 Phase II/III combination drug trials to be launched by the end of 2019.

AhR is a druggable transcription factor that acts as a master regulator of the immune system. The enzymes IDO1, IDO2 and TDO are frequently overexpressed in numerous tumour types and convert tryptophan into kynurenine (KYN) in the tumour microenvironment. KYN is then actively transported into dendritic cells and effector T-cells that are mobilised to detect and kill tumour cells. KYN signalling via AhR in these cell types converts them into regulatory T-cells, suppressing the immune system and preventing it from attacking tumour cells. Research has demonstrated

that the unique advantages of AhR antagonists include broadly inhibiting the signalling of all AhR ligands1 produced by any enzyme that metabolises tryptophan, and robust activation of the immune response to kill cancer cells.

Actinium is developing its Iomab-ACT program to be a universal lymphodepletion regimen to replace chemotherapy-based regimens such as Flu/Cy (Fludarabine and Cyclophosphamide) that are used in standard practice today. Actinium proposes that Iomab-ACT has a multi-modal mechanism of action that can; 1) deplete lymphocytes to create a suitable homeostatic cytokine environment; 2) deplete immune suppressive cell populations that may hinder activation of CAR-T cells; (3) deplete macrophages that may secrete cytokines implicated in CRS and neurotoxicity; and (4) potential anti-tumor effect on CD45+ blood cancer cells.

Iomab-ACT is an ARC or Antibody Radiation-Conjugate that targets the antigen CD45, which is uniquely expressed on leukemia, lymphoma and immune cells making it an ideal target for targeted condition prior to CAR-T and adoptive cell therapies. Iomab-ACT is a lower, outpatient, non-myeloablative dose of Actinium’s lead program, Iomab-B, which has been studied in over 300 patients and is currently being studied as a targeted conditioning agent prior to bone marrow transplant in a pivotal Phase 3 trial.

Actinium presented initial feasibility data for its ACT program at the Transplantation and Cellular Therapies Meeting in February 2019. The data demonstrated that a CD45 targeting antibody labeled with the radioisotope Iodine-131 effectively depleted greater than 90% of lymphocytes and other immune cells while sparing platelets, neutrophils and bone marrow stem cells in preclinical animal models (Click here for TCT poster). Additionally, in a preclinical model of adoptive cell therapy, the CD45-targeted ARC enabled improved tumor control. Actinium also presented data at the Society of Nuclear Medicine and Molecular Imaging demonstrating that the Iomab-ACT program could utilize the radioisotope Lutetium-177 with an anti-CD45 antibody to achieve targeted lymphodepletion prior to adoptive cell therapy (Click here for SNMMI poster).
